SECTION B - PARENTAL CONSENT

SAFETY & RISK DECLARATION

I have considered and understand the nature of the activities offered by Kids Nerf Parties UK and I accept that these activities can expose participants to many risks and can cause physical injury. I have discussed participating in these activities with the child (as named above) and I give my consent for the child to take part in these activities. *

SECTION C: CONDITIONS OF USE

I acknowledge and accept that Nerfday Boys Ltd (‘Kids Nerf Parties UK’) shall not be liable for;any damage or loss of any property or items belonging to my child, group, or any third parties personal injury or death to my child, group, or any third parties (but only so far as such injury or death is not caused by 'Kids Nerf Parties UK' negligence).

I acknowledge and accept that any information that I supply will be treated in accordance with the provisions of the Data Protection Act 1998. Kids Nerf Parties UK may carry out photography and or video recording for publicity purposes, including reproduction on our website and on social networking sites. By accepting these conditions you agree that Kids Nerf Parties UK and any authorised party may use such images in perpetuity and in any format whatsoever. You further agree that copyright in these materials rest with Kids Nerf Parties UK.

I accept and have impressed upon my child that they must comply at all times fully and promptly with all the safety regulations and instructions issued by Kids Nerf Parties UK staff. In participating in a Kids Nerf Parties UK event my child agrees to follow the directions and orders of any member of the Kids Nerf Parties UK staff and I have explained this to my child. Kids Nerf Parties UK reserves the right to exclude any person(s) whom it reasonably considers is in breach of these conditions. Kids Nerf Parties UK will not accept liability for any expenses, claims, losses or costs incurred as a result of such exclusion. I accept and have impressed upon the child that they must not do anything (or omit to do anything) which may cause damage or loss to property or facilities of our venues, to other Participants or cause nuisance, annoyance, disturbance, inconvenience or injury to any other persons or third parties on the premises.
