SECTION B - PARENTAL CONSENT

SAFETY & RISK DECLARATION

I have considered and understand the nature of the activities offered by Bubble Boys Ltd and I accept that these activities can expose participants to many risks and can cause physical injury. I have discussed participating in these activities with my child (as named above) and I give my consent for my child to take part in these activities. *

SECTION C: CONDITIONS OF USE

I acknowledge and accept that Bubble Boys Ltd (‘BBL’) shall not be liable for;any damage or loss of any property or items belonging to my child, group, orany third parties personal injury or death to my child, group, or any third parties (but only so far as such injury or death is not caused by ‘BBL’s negligence).

I acknowledge and accept that any information that I supply will be treated in accordance with the provisions of the Data Protection Act 1998. BBL may carry out photography and or video recording for publicity purposes, including reproduction on our website and on social networking sites. By accepting these conditions you agree that BBL and any authorised party may use such images in perpetuity and in any format whatsoever. You further agree that copyright in these materials rest with BBL.

I accept and have impressed upon my child that they must comply at all times fully and promptly with all the safety regulations and instructions issued by BBL staff. In participating in an BBL event my child agrees to follow the directions and orders of any member of the BBL staff and I have explained this to my child. LBFC reserves the right to exclude any person(s) whom it reasonably considers is in breach of these conditions. BBL will not accept liability for any expenses, claims, losses or costs incurred as a result of such exclusion. I accept and have impressed upon my child that they must not do anything (or omit to do anything) which may cause damage or loss to property or facilities of our venues, to other Participants or cause nuisance, annoyance, disturbance, inconvenience or injury to any other persons or third parties on the premises.
